Custom winform for probe screen

Is there anyone who is for hire to make a custom winform pop up? I have been using the probing macro originally developed by John Revill I believe and customized NEON screen set. I would like to use a custom form pop up in place of the current generic winform that pops up for user to put in the spoil board or material thickness. I have a form I designed in visual studio but lack the knowledge to implement. I would gladly pay someone to look over and change. Any interest from board members? Attached png example of screen i would like in place of the generic winform, also attached. I cannot guarantee the form is correctly made in visual studio since I am not fluent in any of this.

I know enough to hack things together and that's about it. I have tried to learn but it is just beyond my capabilities.
